With Initial post hospital visits, an up to 60-minute at-home visit will be scheduled, and you will be given access to a patient portal.
We will send you the intake documents needed for the initial visit either through your patient portal or by encrypted email. The completed intake documents, your contact information and past records (if available) must be submitted through your portal or by email prior to the initial visit if possible.

Follow-up visits will last between 35 minutes and 45 minutes.

Yes, we accept Medicare, Medicaid, and some other commercial insurances.
We also accept cash and credit cards payments.
Visits vary, initial visits are up to 30- 60 mins and follow up visits are 15-30 mins.
Absolutely your PCP knows you the best, our role is to be your interim provider while you are waiting to see your PCP after you are released from the hospital.
No, we provide mobile services, so we come to you.

Appointments must be cancelled 72 hours prior to the appointment time or rescheduled within the same work week if open slots are available to avoid paying a cancellation fee.

The cancellation fee is the cost of the session due at the time of the missed sessions.

Multiple no-shows or late cancelations may result in discharge from the practice.

You are responsible for this fee regardless of whether you receive a reminder of your appointment.

The preferred method of NON-URGENT communication is by the patient portal or email. It may be used for scheduling and refill requests, records requests, and non-urgent medical issues. Messages are generally responded to on the next business day.

If I am out of the office and additional delays are expected, you will receive an auto-reply.

If you are experiencing chest pain, confusion or other symptoms of a possible heart attack or stroke, or any other emergency condition, call 911 or go to your nearest emergency department immediately. The emergency room should then contact your primary care physician.
After hospital discharge patients should see their primary care provider within a week for follow you. With Post-Hospitalization Visits our goal is to see patients in their home setting within 24-72hrs to provide that at home primary care follow up.
